Antibiotics for acute infective conjunctivitis in children.

چکیده
Peter Rose and colleagues (July 2, p 37) compared the efficacy of chloramphenicol eye drops with placebo eye drops for the treatment of acute infective conjunctivitis in children. They found that “most children presenting with acute infective conjunctivitis in primary care will get better by themselves and do not need treatment with an antibiotic”. Their recommendations should represent a major sea change in management, but we have some reservations. The trial is subject to selection bias since only an estimated 29% of all children presenting in the study period were recruited. There is evidence that substantial numbers of children with more severe conjunctivitis did not enter into the randomisation stage, and these are the very children who were likely to benefit the most from antibiotic treatment. Furthermore, since chloramphenicol reduces the number of pathogenic bacteria in the eye, failure to treat could result in a higher risk of transmission. Without antibiotic treatment, children would need to be isolated from their peers for longer, causing greater inconvenience and possibly a loss of earnings for the family. Had an objective primary outcome measure been used, independent of the children’s parents, with stratification of conjunctivitis by severity, we feel the study findings would have been more robust.

BACKGROUND Acute Infective Conjunctivitis may be due to viral or bacterial infection; though it is usually self limiting, topical antibiotics are often prescribed for rapid recovery. OBJECTIVES This study aims to find out prevalence of bacterial infection among cases of acute infective conjunctivitis and to determine their in-vitro antibacterial susceptibilities to commonly used antibacterial...

INTRODUCTION There is a shifting trend in susceptibility and resistance of the bacteria towards available antibiotics in the last decade. Therefore, periodic studies to monitor the emerging trends in antibiotic susceptibility and resistance are crucial in guiding antibiotic selection. OBJECTIVES The aim of this study was to determine the most common pathogens causing bacterial conjunctivitis,...

BACKGROUND Acute infective conjunctivitis is common among preschool children. Public Health England (PHE) recommends that children with conjunctivitis do not need to be excluded from child care, but childcare providers are required to determine their own sickness policies and prior research suggests that children are often excluded until they are treated or have recovered. How the content of th...
متن کاملAcute infective conjunctivitis in primary care: who needs antibiotics? An individual patient data meta-analysis.
BACKGROUND Acute infective conjunctivitis is a common problem in primary care, traditionally managed with topical antibiotics. A number of clinical trials have questioned the benefit of topical antibiotics for patients with acute infective conjunctivitis. AIM To determine the benefit of antibiotics for the treatment of acute infective conjunctivitis in primary care and which subgroups benefit...
